Referring to nephritis, we usually think of glomerulonephritis. Because for the majority of renal patients, the location of lesion is glomerulus.
There are two main manifestations of glomerular disease.
1. Hematuria. Different forms of red blood cells will be found in urine sediment microscopy, such as acanthocyte, red blood cast, etc.
2. Proteinuria. If your 24h urine protein is over 3.5g per day, it must be kidney disease. If your 24h urine protein is over 3.5g per day, and serum albumin is less than 30g/L, it must be Nephrotic Syndrome.
For patients with glomerular disease, some have proteinuria or hematuria independently, while some have both. Besides they may have high blood pressure, swelling, decreased renal function, etc.
In western medicine, renal biopsy is a way to diagnose glomerular disease, but it can do harm to kidneys, especially for child patients. Renal biopsy may affect their growth and development. Clinically doctors can refer to the key indicators such as the degree of urinary protein, urinary sediment, age of onset and so on to confirm the pathological type of kidney disease, narrow the scope of diagnosis and avoid renal biopsy.
1. Obvious blood urine, while proteinuria is not obvious. No Nephrotic Syndrome and renal insufficiency. Those manifestations belong to the following pathological types:
(1)IgA Nephropathy, Purpura Nephritis, thin basement membrane nephropathy, mesangial proliferative glomerulonephritis, hereditary nephritis, etc. Patient age is usually below 15 years old.
(2)This Basement Membrane Nephropathy, hereditary nephritis, lupus nephritis, mesangial proliferative glomerulonephritis, IgA Nephropathy, etc. Age is 15-40 years old.
(3) IgA Nephropathy. Age is over 40 years old.
2. Obvious hematuria and proteinuria. Some present Nephrotic Syndrome and renal insufficiency. Illness conditions are serious. If so, you are likely to be the following pathological type.
(1) Membrano-proliferative glomerulonephritis, postinfectious glomerulonephritis. Age is below 15 years old.
(2) Rapidly progressive glomerulonephritis, lupus nephritis, membrano-proliferative glomerulonephritis, postinfectious glomerulonephritis. Age is 15-40 years old.
(3) Vasculitis, rapidly progressive glomerulonephritis, postinfectious glomerulonephritis. Age is over 40 years old.
3. It is mainly involved in the clinical manifestations of Nephrotic Syndrome. Hematuria is no more the key observation index.
(1)FSGS, Minimal Change Disease, Mesangial proliferative glomerulonephritis. Age is usually under 15 years old, especially minimal change disease patients.
(2)Membranous Nephropathy (including lupus nephritis), FSGS, postinfectious glomerulonephritis, Diabetic Nephropathy, Minimal Change Disease. Age is 15-40 years old.
(3) IgA Nephropathy, Diabetic Nephropathy, Primary systemic amyloidosis (age is over 60 years old), FSGS, Membranous Nephropathy, Minimal Change Disease, postinfectious glomerulonephritis. Age is over 40 years old.
After such a screening, the scope of kidney disease will be significantly reduced. If you combine medical history and other serological examinations, the diagnosis of disease will be more accurate, meanwhile you can avoid renal biopsy.
