1. Decreased glomerular filtration rate (GFR): For example, in acute glomerulonephritis, inflammatory exudates and endothelial cell swelling narrow or occlude the glomerular capillary lumen, reduce glomerular blood flow and decrease filtration rate; in chronic glomerulonephritis, due to a large number of glomerular fibrosis, the glomerular filtration area reduces so as to affect its filtration rate, which is the main mechanism of glomerular edema.
2. Anaphylatoxin: the immune damage in glomerulonephritis activates complement to produce anaphylatoxin, which increases the permeability of capillaries throughout the body, resulting in a large amount of water and plasma protein penetrating into the interstitial tissue and causing edema.
3. Cardiac insufficiency: Renal disease accompanied by hypertension, anemia, electrolyte disturbance can cause cardiac insufficiency so as to aggravate edema, which is also one of the causes of renal edema.
4. Increase of secondary aldosterone: Because of the decrease of renal blood flow and the increase of renin secretion in renal lesions, secondary aldosterone secretion increases, resulting in increased sodium reabsorption in renal distal convoluted tubules so as to cause edema.
5. Decrease of plasma colloid osmotic pressure: In Nephrotic Syndrome, excessive loss of plasma proteins, especially albumin, is caused by large amounts of proteinuria, resulting in a decrease of plasma colloid osmotic pressure. When albumin is lower than 25g/L, it can cause systemic edema and even abdominal or pleural effusion.
6. Glomerulotubular imbalance: GFR decreases in renal diseases, but renal tubule damage is mild, which can maintain more water and sodium reabsorption. For example, in Nephrotic Syndrome, because of water and sodium elimination disturbance, glomerulotubular imbalance and the redistribution of renal blood flow, the activity of renal nerve is enhanced, and the production of natriuretic hormone is inhibited, which makes the renal tubules increase the reabsorption of sodium and water so as to aggravate edema.
